   Diabetes Care Team       Diabetes Support Program

  • The Diabetes Care Team is for any patients living in the Niagara Region with Type 2 diabetes or Pre-diabetes
  • The Diabetes Support Program is strictly for Niagara North Family Health Team patients of the Virgil and/or Niagara-on-the-Lake sites.

 

Diabetes Care Team – St. Catharines

Our Diabetes Support Program is facilitated by our Registered Dietitians, and our Registered Nurses, who are Certified Diabetes Educators.

The Niagara North FHT coordinates a group of healthcare providers available to address any diabetes concerns you may have.

Program services:

  • Individual assessment and counseling is provided to help you work towards a plan that allows you to control your diabetes and prevent its complications.
  • Ongoing follow up support for clients who require more individual support for their self-care management is available.
  • Support is provided for those who require insulin to show you how to use it and how it will help improve your blood sugar levels.
  • For patients of our Diabetes Care Team we offer a Living Well with Diabetes workshop where participants will be given general information on diabetes and its management
  • We also offer a Physical Activity Classfacilitated by a Registered Kinesiologist.

How to get involved:

In order to be seen, clients must either fill out a self-referral form or be referred by their physician.

This program is open to any adults in Niagara Region with Type 2 Diabetes or who have been diagnosed with pre-diabetes.


Diabetes Care Team: Diabetic Foot Assessments – St. Catharines

We also offer diabetic foot assessments for patients of the Diabetes Care Team.

Appointments will include:

  • diabetic screening of the nerves and blood vessels
  • preventative diabetic foot-care advice
  • referrals made if necessary

*Please note foot care will not be provided at these appointments – assessment only.

When it’s offered:

  • Tuesday February 19, 2019 from 9:00am – 12:00pm (by appointment)
  • Tuesday May 7, 2019 from 1:30pm – 3:30pm (by appointment)
  • Tuesday Sept 25, 2019 from 1:30pm – 3:30pm (by appointment)

Diabetes Care Team: Physical Activity Class – St. Catharines

We also offer a Physical Activity Class for diabetic or prediabetic patients of the Diabetes Care Team.

Participants will:

  • learn from a Registered Kinesiologist about the importance of physical activity in the prevention and management of diabetes
  • receive a resistance band and be given tips and guidance about how to effectively utilize it in their daily exercise routines

  • Nov 14, 2019

All classes run from 1:30pm – 3:30pm at our 22 Ontario Street location (McMaster Niagara Family Health Centre).

This program is open to any adults in St. Catharines – Thorold area with Type 2 Diabetes or who have been diagnosed with pre-diabetes.

Diabetes Support Program – Niagara-on-the-Lake

This program is for you if:

You have Type 2 diabetes or are at risk for developing diabetes.

What the Diabetes Support Program has to offer:

In this Shared Medical Appointment, a Medical Doctor, a Registered Nurse and a Registered Dietitian all work together to help you live well with diabetes. This is a great opportunity to learn in an interactive group setting with other people with diabetes who are interested in improving their health.

The Diabetes Support Program provides opportunities to learn more about:

  • Healthy Eating
  • Physical Activity
  • Blood sugar monitoring
  • Diabetes medications and insulin
  • Preventing and managing diabetes-related complications
