Before completing patient profile forms, please ensure that you meet the following criteria:
- You are not already rostered with one of our 5 physicians at the Virgil site of Niagara North Family Health Team.
- You live in Niagara-on-the-Lake, Virgil, St. David’s or Queenston and do not have a doctor.
- Please note there is a WAIT LIST for new Family Doctor applications.

Please note there are currently no physicians accepting new patients at the St. Catharines sites of the NNFHT.
Niagara-on-the-Lake
