What is an Outreach Coordinator?
Outreach Coordinators are trained Registered Nurses who help connect patients with programs and services at Niagara North Family Health Team and/or resources and supports that are available in the community.
What is the role of the Outreach Coordinators in the Niagara North Family Health Team?
Our Outreach Coordinators have a large focus on providing support to patients who have recently been discharged from the hospital. Our goal is to optimize our at-risk patients health and safety in their homes and decrease unnecessary readmission to hospital.
Who should be referred to the Outreach Coordinator:
- Individuals who may need assistance with accessing programs and services at the Niagara North Family Health Team.
- Individuals who are unsure which supports and resources are available to them both within the NNFHT and in the broader community.
