   Diabetes Care Team   Diabetes Support Program Keeping on Track With Diabetes Group Pre-Diabetes Group Physical Activity Class

  • The Diabetes Care Team is for any patients living in the Niagara Region with Type 2 diabetes or Pre-diabetes
  • The Diabetes Support Program & the Keeping on Track with Diabetes Group program is strictly for Niagara North Family Health Team patients of the Virgil and/or Niagara-on-the-Lake sites.

Diabetes Care Team – St. Catharines

Our Diabetes Care Team is facilitated by our Registered Dietitians, and our Registered Nurses, who are Certified Diabetes Educators.

The Niagara North FHT coordinates a group of healthcare providers available to address any diabetes concerns you may have.

Program services:

  • Individual assessment and counseling is provided to help you work towards a plan that allows you to control your diabetes and prevent its complications.
  • Ongoing follow up support for clients who require more individual support for their self-care management is available.
  • Support is provided for those who require insulin to show you how to use it and how it will help improve your blood sugar levels.
  • For patients of our Diabetes Care Team we offer a Living Well with Diabetes workshop where participants will be given general information on diabetes and its management
  • We also offer a Physical Activity Classfacilitated by a Registered Kinesiologist.

How to get involved:

In order to be seen, clients must either fill out a self-referral form or be referred by their physician.

This program is open to any adults in Niagara Region with Type 2 Diabetes or who have been diagnosed with pre-diabetes.

COVID-19 UPDATE:

The risks that COVID-19 present means that the majority of of visits have and will continue to be done virtually either over the phone whenever possible.

During the COVID-19 pandemic our Diabetes Care Team visits will be offered primarily over the phone with some in-person visits, when indicated by the healthcare provider.


Diabetes Care Team: Physical Activity Class – St. Catharines

We also offer a Physical Activity Class for diabetic or prediabetic patients of the Diabetes Care Team.

Participants will:

  • learn from a Registered Kinesiologist about the importance of physical activity in the prevention and management of diabetes
  • receive a resistance band and be given tips and guidance about how to effectively utilize it in their daily exercise routines

Diabetes Support Program – Niagara-on-the-Lake

This program is for you if:

You have Type 2 diabetes or are at risk for developing diabetes.

We recommend that you check out our “Keeping on Track with Diabetes” virtual group program, however, we also offer individual appointments with the Diabetes Support Program Registered Dietitian, Registered Nurse and our team Pharmacist.

Keeping on Track with Diabetes – Group Program

This program is for you if:

You have Type 2 diabetes or are at risk for developing diabetes.

What the Keeping on Track with Diabetes Group Program has to offer:

In this virtual group, a Medical Doctor, a Registered Nurse and a Registered Dietitian all work together to help you live well with diabetes. This is a great opportunity to learn in an interactive virtual group setting with other people with diabetes who are interested in improving their health.

During the COVID-19 pandemic our Keeping on Track with Diabetes group will be offered virtually using the Zoom platform. For participation in this group an e-mail address and a device (computer, phone, tablet) with audio and video capability are required.

Pre-Diabetes Group

This program is for you if:

Are at risk for developing diabetes.

What the Pre-Diabetes Group Program has to offer:

This 2-hour workshop is designed to help patients with pre-diabetes begin engaging in diabetes prevention. Our Registered Dietitian & Certified Diabetes Educator will help patients understand what pre-diabetes is, their risk of developing diabetes, and what steps can be taken to prevent developing diabetes.

This is a great opportunity to learn in an interactive virtual group setting with other people with pre-diabetes who are interested in improving their health.
