Many of the physicians, nurse practitioners and physician assistants at the Niagara North Family Health Team offer our secure web based portal called Pomelo (previously known as Health Myself).
Our portal offers a variety of features to enhance your experience as a patient, such as:
- securely send and receive electronic messages and attachments to/from our staff
- receive important updates from our team by e-mail
- receive appointment reminder emails
- book physician, nurse practitioner and physician assistant appointments online
How do I Register?
REGISTER for Pomelo
Registering your e-mail with the Niagara North Family Health Team will give you access to our secure web-based patient portal called Pomelo (previously known as Health Myself).
After we receive your completed e-mail consent form, we will e-mail you a link to register your account with the Pomelo portal.
If you do not receive the registration link within 72 business hours, please double-check your junk or spam folders. Contact the clinic directly if you do not receive the link after 72 hours and it is not in your junk or spam folders.

PORTAL HOW TO’S:
- How do I connect my children to my Pomelo Portal account?
After registration, patients may manually add their family members through the Connected Accounts option under the My Account menu found at the top right-hand corner of their Pomelo account.
- How do I send a message about my children to the clinic?
After you have successfully connected your children to your existing Pomelo account, you can Compose a New Message and change who the conversation is about by selecting from your connected account options in the compose a new message area.
- Can I access the portal if I am not a patient myself, but I am a caregiver/parent/guardian for a patient of the NNFHT?
Yes! Please contact your clinic directly and request a “Caregiver Account” be created for you as long as there is documented POA for healthcare purposes on file with the clinic or for children under 16 who are patients of NNFHT with parents/guardians who have a doctor outside of the NNFHT.
