Aging at Home
The goal of the Aging at Home program is to provide care to patients over age 65 with complex medical and/or social conditions who have difficulty accessing regular care at the clinic by providing home visits.
ABOUT THIS PROGRAM:
This newer team-based program currently involves Nurse Practitioners, Community Outreach Nurses, and Occupational Therapists.
HOW TO GET INVOLVED:
Currently only available at our Niagara on the Lake Site by physician or Nurse Practitioner referral.
