Aging at Home

The goal of the Aging at Home program is to provide care to patients over age 65 with complex medical and/or social conditions have difficulty accessing regular care at the clinic by providing home visits.

ABOUT THIS PROGRAM:

This newer team-based program that currently involves Nurse Practitioners, Community Outreach Nurse, Pharmacist and Occupational Therapist.

HOW TO GET INVOLVED:

Currently only available at our Virgil Site by physician or Nurse Practitioner referral.

COVID-19 UPDATE:

The risks that COVID-19 present means that the majority of of visits have and will continue to be done virtually either over the phone whenever possible.

During the COVID-19 pandemic our Aging at Home visits will be offered over the phone with some in-person home visits, at the discretion of the healthcare providers.
