Live Heart Smart

What the LIVE HEART SMART Program has to offer:

In this Shared Medical Appointment, a Medical Doctor, a Registered Nurse and a Registered Dietitian all work together to help you live heart smart. This is a great opportunity to learn in a group setting with other people who have high cholesterol and are interested in improving their heart health.

Second Tuesday of every month.

This group is only open to patients of Niagara-on-the-Lake Hospital Site and the Virgil Site.

Hypertension Consultation

This program is run by our Registered Nurse Educators.

This program is for you if

  • You have been told that you have high blood pressure
  • You are interested in preventing high blood pressure
  • You would like to learn how to measure your blood pressure
  • You would like to know more about lifestyle effects on blood pressure

What the hypertension program has to offer

  • You will learn about blood pressure and risks associated with high blood pressure.
  • You will learn about your numbers, and what you can do to prevent high blood pressure
  • You will how to measure your blood pressure at home
  • You will learn about the links between blood pressure and sodium, physical activity, healthy eating, stress effects and coping strategies
  • You will learn about medications used to treat blood pressure

When it’s offered

Currently the program is offered on an individual basis by appointment.

Plans are in progress to provide group education sessions about high blood pressure and heart health. Please check back for updates on the group program.
